Position Summary Reporting to the Clinical Manager, the Nurse Led Outreach Team (NLOT) Nurse provides clinical consultation, education, and mentorship to interprofessional teams within Long Term Care Homes (LTCHs). This role supports the delivery of high-quality, evidence-based geriatric care to improve resident outcomes, reduce avoidable emergency department visits and hospital admissions, and support timely transitions back to LTC. The NLOT Nurse delivers mobile clinical assessment and intervention for residents experiencing acute or episodic changes, including functional decline. Working collaboratively with Physicians, Nurse Practitioners, Nurses, geriatric teams, Ontario Health at Home, residents, families, and community partners, this role supports complex geriatric care, including frailty, dementia, chronic disease, and palliative/end-of-life care. The role focuses on capacity building within LTCHs through education, coaching, and implementation of clinical best practices, while facilitating access to appropriate community and ambulatory services to ensure coordinated, resident-centred care. Primary Responsibilities Collaborate to enhance the return of residents to LTC homes following unavoidable ED visits, providing additional support and mentorship to LTC staff as needed. Provide comprehensive assessment, intervention, monitoring, and follow-up care for acute and chronic illnesses. Deliver best practice education and support to caregivers, families, and LTC home staff, as well as other stakeholders upon request. Coordinate care and promote excellence in patient-centered nursing through expert practice, education, consultation, and research. As a member of the inter-professional team, the NLOT RN will practice in accordance with relevant regulatory acts and standards, including the Regulated Health Professions Act, the Gerontological Nurses Association (GNA) Standards, the Long-Term Care Homes Act, and the Practice Standards of the College of Nurses of Ontario. The NLOT RN will implement and maintain effective programs to reduce acute care admissions from the LTC sector, emphasizing wellness, disease prevention, health promotion, staff mentoring, outreach, advocacy, and community development. Program development will align with community needs, hospital priorities, and guidelines set by Ontario Health, the Ministry of Health, and the Ministry of Long-Term Care. Mandatory Qualifications Baccalaureate Degree in Nursing Registered Nurse (RN) in good standing with the College of Nurses of Ontario Minimum 3 years recent clinical experience in acute medicine and geriatrics Current Basic Cardiac Life Support (BCLS/CPR) certification Geriatric Certificate Program (GCP) required Certification or training in GPA (Gentle Persuasive Approach), or behavioural management approaches Advanced clinical skills including, but not limited to IV therapy, wound care/pressure injury prevention, drain management etc. Valid Ontario driver’s license and access to a reliable vehicle Core Competencies Demonstrated clinical expertise in geriatric care, including frailty, dementia, chronic disease, complex medical and palliative care Strong assessment, problem recognition and clinical decision-making skills with ability to respond to acute changes in condition Ability to provide consultation, mentorship, and coaching to interprofessional teams Experience supporting practice change through education and knowledge translation Skilled in care coordination and system navigation, including linking to community and ambulatory services Strong communication, collaboration, and relationship-building skills Demonstrated leadership, critical thinking, and problem-solving abilities Ability to work independently with strong organizational and time management skills Proficiency with computer systems and documentation practices Knowledge of the healthcare system across acute, LTC, and community sectors Commitment to quality improvement, evaluation, and patient safety Ability to maintain confidentiality and comply with privacy and safety legislation Preferred Qualifications Experience in program development and quality improvement Training in Comprehensive Geriatric Assessment (CGA) Familiar with the Long-Term Care Act 2021 Wound care certification or advanced knowledge Membership in professional organizations (e.g., RNAO) Experience in outreach or mobile care models Our Commitment to Equity, Diversity, and Inclusion BCHS is committed to building an inclusive and accessible workplace that reflects the diversity of the communities we serve. Requirements
