Founded by The Kitchener Downtown Community Health Centre and Sanctuary Refugee Health Centre, our services are responsive to individual health goals, and together we create healthy communities. We offer evidence-informed, innovative healthcaring for anyone facing barriers to health services, including people who are newcomers, refugees, experiencing homelessness or precarious housing and who are challenged by other social influences of health in the Kitchener-Waterloo region.

The Nurse Practitioner provides comprehensive primary care, including diagnosis, treatment, and health education within an interdisciplinary team. They also participate in health promotion programs, quality improvement initiatives, and maintain accurate clinical records.

Registered Nurse; Diabetes Education Program

The Registered Nurse provides community-based diabetes education, insulin management, and behaviour change support to clients within an interdisciplinary team. They also participate in the development and evaluation of diabetes programs while supervising students and volunteers.
