Responsibilities include conducting advanced health assessments, developing differential diagnoses, ordering and interpreting diagnostic tests, prescribing and managing pharmacological and therapeutic interventions, and delivering patient-centered care. The role collaborates with clients, families and interdisciplinary health care providers to develop and implement holistic care plans, including prenatal support for at-risk youth. The Nurse Practitioner facilitates a case management approach, supports individuals and communities in addressing health determinants and partners with health care and community organizations to improve outcomes for at-risk populations. The position also contributes to quality improvement initiatives and the enhancement of service delivery models.
