Position #: 9007916 Expected Start Date: To be Determined; Posted until Filled Union: SUN Facility: Weyburn Primary Health Care City/Town: Weyburn Department: Primary Care Sites Type: Full-time regular FTE: 1 Shift Information: Days, Standby Hours of Work: 14 shifts of 8 hours per 3 weeks Relief: No Float: No Field Hours: Yes Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910 Travel Required: Yes Job Description: In collaboration with the Primary Health Care Teams, physicians and health care provider team members, the Registered Nurse (Nurse Practitioner) will provide Primary Health Care (PHC) services to assigned populations which may include: urban populations, visiting services to rural communities, and/or priority populations (e.g. newcomers, teens, residents in long term care facilities) as determined by community and program needs. Depending on community and team requirements, the position may include first on-call support. As the PHC Team evolves, and based on needs of the population, the position may be required to provide new practices such as nurse led extended hour clinic services and/or first on-call support. Primary Health Care services include: health assessment, health promotion and education, disease and injury prevention and assessment, treatment interventions; collaboration with, and referral to, other health care providers; participation in Lean, patient safety, community development, clinical practice redesign, quality improvement, and evaluation. The Nurse Practitioner will provide primary health care services to assigned populations, including health assessments, disease prevention, and treatment interventions. They will collaborate with multidisciplinary teams and participate in quality improvement and clinical practice redesign initiatives.

Candidates must hold a Master of Nursing - Nurse Practitioner degree and possess at least three years of nursing experience. A valid Class 5 driver's license and relevant certifications such as BLS-C and ACLS are required.
