They collaborate with an inter-professional team to ensure person-centered care and effective discharge planning. Candidates must have graduated from an approved School of Nursing and hold current practicing registration with the BCCNM. Two years of recent clinical nursing experience in the designated area and completion of relevant post-graduate specialty education are required.
