Collaborates with an inter-professional team to facilitate client transitions and ensure continuity of care across the healthcare continuum. Requires current registration as a Registered Nurse with the British Columbia College of Nurses and Midwives (BCCNM). Candidates must have at least one year of recent clinical experience in acute or urgent care settings.
