The Community Health Nurse provides independent clinical care, assessments, and interventions for clients in their homes to support chronic disease management and recovery. They collaborate with interprofessional teams to develop care plans and facilitate transitions across the healthcare continuum. Candidates must hold current practicing registration as a Registered Nurse with the BCCNM and possess at least one year of recent clinical experience with complex geriatric or adult patients. A valid BC driver's license and access to a personal vehicle are also required.
