They emphasize health promotion, chronic disease management, and facilitate transitions across the healthcare continuum to ensure safe care at home. Candidates must hold a current practicing registration as a Licensed Practical Nurse with the BCCNM and possess at least one year of recent related clinical experience. A valid BC driver's license and access to a personal vehicle are also required for this role.
