The Community Health Nurse provides direct clinical care, assessments, and care planning for clients with complex chronic conditions in a community setting. They collaborate with interprofessional teams and primary care providers to facilitate transitions and promote health maintenance for clients living at home. Candidates must hold current registration as a Licensed Practical Nurse with the BCCNM and possess at least one year of recent clinical experience with geriatric or adult patients. A valid BC driver's license and access to a personal vehicle are also required for this position.
