The Community Health Nurse provides independent clinical care, assessments, and health coaching to clients in their homes and community settings. They collaborate with interprofessional teams to manage complex chronic conditions and facilitate smooth transitions across the healthcare continuum. Candidates must hold current registration as a Licensed Practical Nurse with the BCCNM and possess at least one year of recent clinical experience with geriatric or adult patients. A valid BC driver's license and access to a personal vehicle are also required for this position.
