The Community Health Nurse works independently and collaboratively to manage an assigned client caseload, providing assessments, interventions, and care planning. They facilitate health recovery and transitions across the healthcare continuum while providing direct care and education to clients and their families in home and community settings. Candidates must hold current practicing registration as a Licensed Practical Nurse with the BCCNM and possess at least one year of recent clinical experience with complex geriatric or adult patients. A valid BC driver's license and access to a personal vehicle are also required for this position.
