They collaborate with interprofessional teams to facilitate care transitions and ensure clients can live safely and confidently at home. Candidates must hold current practicing registration as a Licensed Practical Nurse with the BCCNM. The role requires at least one year of recent clinical experience in acute or community settings and a valid BC driver's license.
