Nurse Practitioner - HART Hub Dufferin

The Nurse Practitioner will assess and treat symptoms of substance use, facilitate connections to the continuum of care, and work closely with the Rapid Access Addiction Medicine (RAAM) team as an integrated member of the HART Hub. Responsibilities include attending to clients with addictions and concurrent disorders both in the clinic and through community home visits, performing comprehensive health assessments, and ordering/interpreting diagnostic tests.
