The Complex Continuing Care program cares for older adults with multiple chronic health conditions. We provide ongoing in-hospital care that aims to improve the quality of life of medically complex patients. Job Summary The Complex Continuing Care Unit provides care for older patients with complex medical conditions in a non-acute hospital setting. The Complex Continuing Care Program helps patients and their families identify strategies that support quality-of-life goals as well as short term and long term care needs. Key Responsibilities Provides sound clinical knowledge and assessment skills, acts as clinical expert and resource and provides comprehensive, quality client/family centred health care services from admission, through transfers and discharge Plans for, delivers and evaluates nursing interventions, including medications, as well as treatments which fall within the RN’s scope of practice Provides client-centred care within interdisciplinary care team to meet the personal care, activation, recreation and spiritual needs of clients Participates actively in the teaching programs for nurses and nursing students Qualifications Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required BScN preferred Demonstrated ability to take professional accountability for leading, organizing, providing and delegating care activities for clients and families Experience in caring for geriatrics patients with complex medical care needs preferred Psychogeriatric experience, i.e. mental health assessment and intervention and working with responsive behaviours preferred Excellent interpersonal, communication and problem-solving skills Basic computer skills – Meditech, Microsoft Word, Outlook, and Excel preferred

Requirements
Requires current registration in good standing with the College of Nurses of Ontario and a valid Basic Cardiac Life Support certificate. A Bachelor of Science in Nursing is preferred, along with experience in geriatric or psychogeriatric care.
